Many grateful families, friends and former patients have provided for the Children's Hospital Foundation or Norton Healthcare Foundation as part of their estate. Bequests enable Norton Healthcare to provide the best possible care for the patients we serve, expand clinical and research efforts and endow key programs.
For Children's Hospital Foundation (to benefit Norton Children's Hospital and our pediatric services)
"I give, devise and bequeath $________ or ______% of my estate to the Children's Hospital Foundation, a Kentucky non-profit corporation located at 4965 U.S. Highway 42, Suite 1000, Louisville, Kentucky 40222, Federal Tax ID # 61-6027530, to be used at the discretion of the Children's Hospital Foundation board of directors."
For Norton Healthcare Foundation (to benefit our adult services)
"I give, devise and bequeath $________ or ______% of my estate to the Norton Healthcare Foundation, a Kentucky non-profit organization located at 4965 U.S. Highway 42, Suite 1000, Louisville, Kentucky 40222, Federal Tax ID
# 31-0914919, to be used at the discretion of the Norton Healthcare Foundation board of directors."
